1: E.R. will not see you untill they are sure of payments.
Explanation: I already explained above in explaining upashibokas similar concern but I want to add one more thing! Many insurance companies want you to have a prior aprroval from them before you are admitted!(at least within 24 hrs of admission you must notify them or else they will not pay!). So E.R. clerk may be just doing that! letting your insurance company know that you are in E.R. and you may need an admission.And if your condition is not of emergency nature(in E.R triage nurses opinion not acording to your opinion! Because we all think when we or our loved one is sick that it is very urgent and important for immidiate medical attention!) then it is possible that you will have to wait for 1 to 2 hours in E.R!

Nirakar's 2nd concern:
No doctor attends call at nights and weekends!
Explanation: The nature of medical practice here is totally differant than in India. Here we dont have dispenceries of private doctors like in India.Mostly here there is a group practice---more so with specialist doctors.But you will see some Internal Medicine doctors taking their own calls but even with them you may not see them personally.Be it in single doctor practice or group practice,you will be advised to go to E.R.(if your condition warrants that) of the hospital the doctor is affiliated with.So you all should know which hospital is ok to go acording to your insurance company and which hospital your primary physician is affiliated with.If you talk to your doctors partner who is on call, you are right, he may not know in detail about your past history .But he still should be able to help you after you explain him briefly about your problem and past history if any.With the sheer number of patients each doctor sees it is impossible for the doctor to know evrything about his primary patients and about all the patients of all his partners!So you have to take control and give him as much useful information as possible!BE your own advocate!Unfortunately thats the way medical practice is here! Not like in India where your fmily doctor use to know your last 2 generation and your extended family also!So I can give you only one advise---- be your own advocate with doctor on call who is your primary doctors partner!But nowadays all the doctors in your primary doctors group have your information avialable(if you have past history)by logging on to the computer and accessing your data!

Nirakars 4th concern:
My son had UTI and lab gave weekend as a reason for not doing the pathology work on urine!
Explanation: UTI is usually not a critical condition and it can be diagnosed with signs and symptoms. There is a standard antibiotic therapy which many good doctors will and should start even before you get the urine culture and sensitivity results!So it is not necessary to have that done stat!(means urgent) Had your doctor requested a stat urine results that lab might have done the test with extra charge to you! But in any case the results of culture and sensitivity of the urine(which tells which bacterias are growing in urine) takes 24 to 48 hrs anyway!Urine test is just a proof that antibiotic therapy was warranted! otherwise insurance company will not pay the physicians for such treatment claiming that it was not warranted! so if you had to wait till monday it was not such a gross negligence, but your doctor should have explained you that very clearly! I tell all the patients to ask questions! And remember YOU ALL CAN ASK YOUR DOCTOR QUESTIONS TILL YOU GET THE EXPLANATION AND YOU ARE SATISFIED! IF HE/SHE DOESNT EXPLAIN YOU ITS TIME TO LOOK FOR ONOTHER DOCTOR WHO WILL!

And about tooth pain ,you guys are confusing doctors with dentists! I am in cardiology so I cant speak for dentists!

Another case:
In India I read about this case. Even after tubectomy operation, the mother gave birth to a baby. Court has fined doctor under negligence.
Regarding this I was talking with OB/Gyn specialist in Pune, he told me (with reference to some article in British medical journal ... I have not seen or read that articles) this can be rarely possible.

In above case, as per the explanation from gynecologist, is this really possible?

Mukund, good points and thanks for clear answers, that is waht one should expect from medical professional.

In ER if onsite Dr convinced that patient can wait for next 2-3 hrs and he has other emergancy case to handle, he should say so.
Normally in hospital, First nurse comes and do routine check up and tells us to wait for DR. I belive she can explain clearly that you have xyz symptoms, your BP is normal etc and it dosen't matter even if no one attends you for next 3-4 hr. Hospitals lack in this warm response. You will come to know about this only when you meet DR.

You didn't answer my question on Will DR give discout, they offer it to Insurance?

In case of UTI, with primary symptomes and some quick test Dr started Antibitic, that's true, but her statement that time was " I am not sure about this but only urine test can tell you about infection" Finally results were negative, now question in my mind is, then what infection was that? It got cured by antibiotic given before test though. Answer from Dr was it was UTI, now in my mind we are missing some piece of puzzle here. That day, when she was not sure about infection and Son has fever of 104 consistant, how this can not be urgancy?

Other question was inflated cost of medicine and consulting fee that forces you to go for
insurance.

You answered that ER clark verifies and informs Insurance provider that a person is in ER, it is fine up to that point, but when I went first time in ER for my friend, clark started registering his name etc, after that he started asking can you add your parents because if you add them here we have xyz services etc. I strongly belive ER is not a place for marketing, They could have just ask for SSN and do their work in background and start attending patient, all other things could have been discussed after that.
Am I expecting too much?

my experience with the doctors in USA is very good. They all the time explained all my doubts/questions

मी असे ऐकले की हल्ली cesearian cases मध्ये भारतात gynacologist मुद्दाम पडतात. एखादी स्त्रि normal delivery जरी होत असेल तरी dilation period पूर्ण व्हायच्या आताच नाही आता c-section करावे लागेल असे कारण सांगून cesearean करतात. त्यात त्यांना अधिक पैसे मिळतात. हे कितपत खरे आहे मंडळी? खरच असे होत आहे का?

मी तर हल्ली खोट्या bypass सुद्ध होतात असे ऐकले आहे. ते ही एका doctor कडून. नुसति chest pain ची तक्रार घेउन patient आला की मिळाले आयतेच सावज म्हणे!

मी या क्षेत्राचा फ़ार जवळुन अनुभव घेतलाय बी. एक कथा पण लिहिली होती.
माझ्या गुरु डॉ. डहाणुकरहि या अनुभवातुन गेल्या होत्या. त्यानी मला स्वता, एका प्रसिद्ध हार्ट स्पेशालिस्टचा अनुभव सांगितला होता. त्यांच्या पिढीतले डॉक्टर असे नव्हते, पण आता बहुतेक असेच आहेत.
शिवाय आता आलीत ती मेडिक्लेम वाली मंडळी.
त्यामुळे अगदी अपघात झालेला वैगरे नसेल तर कधीहि सेकंड ओपिनियन घेणे चांगले.
